Little Roots Pediatric Therapy is committed to protecting the privacy and confidentiality of personal health information in accordance with Nova Scotia’s Personal Health Information Act (PHIA), applicable professional standards, and occupational therapy ethical obligations.
Little Roots Pediatric Therapy collects, uses, and stores personal health information only for purposes related to providing occupational therapy services, communicating with clients and families, maintaining clinical and financial records, scheduling appointments, processing payments, meeting legal and professional obligations, and supporting safe, effective care.
Personal health information may include information such as a client’s name, contact information, date of birth, relevant health and developmental history, assessment information, treatment notes, goals, progress notes, reports, billing information, and communication related to occupational therapy services.
Little Roots Pediatric Therapy limits the collection, use, and disclosure of personal health information to the minimum amount reasonably necessary for the purpose. Information is not shared with third parties, such as schools, daycares, other health-care providers, insurers, or family members outside the usual consent/decision-making process, unless consent has been provided or disclosure is permitted or required by law.
As a private occupational therapy practice, Little Roots Pediatric Therapy is responsible for the personal health information in its custody or control. Records are stored using secure systems and reasonable administrative, physical, and technical safeguards. These may include password-protected systems, secure electronic records, limited access, and privacy-conscious communication practices.
Clients, parents/guardians, or authorized substitute decision-makers may request access to or correction of a client record. Requests should be made by contacting:

Before releasing a record, Little Roots Pediatric Therapy will take reasonable steps to verify the identity and authority of the person requesting access. Access or correction requests will be reviewed and responded to in accordance with PHIA. In some situations, access may be limited where permitted by law, such as where the record contains another person’s personal health information, legally privileged information, or information that may create a serious risk of harm.
If a client or authorized decision-maker believes that information in the record is inaccurate, incomplete, or not up to date, they may request a correction. Factual errors may be corrected where appropriate. Professional opinions or observations made in good faith may not be changed, but a statement of disagreement may be added to the record where required.
If there is a privacy concern or complaint, please contact Little Roots Pediatric Therapy directly so the concern can be reviewed and addressed. Individuals may also contact the Nova Scotia Information and Privacy Commissioner, also known as the Review Officer, regarding privacy, access, or correction concerns under PHIA.
In the event of a privacy breach involving stolen, lost, or improperly accessed, used, disclosed, copied, or modified personal health information, Little Roots Pediatric Therapy will take reasonable steps to contain the breach, assess the risk, notify affected individuals and/or the Review Officer where required, document the incident, and take steps to reduce the risk of recurrence.
Little Roots Pediatric Therapy does not use client personal health information, photos, testimonials, stories, or identifying details for marketing, social media, fundraising, or commercial promotion without appropriate consent and consideration of applicable privacy, advertising, and professional standards.
By participating in occupational therapy services with Little Roots Pediatric Therapy, clients and families acknowledge that personal health information will be collected, used, and protected as described above for the purpose of providing occupational therapy services and meeting legal and professional obligations.
